On August 2nd, 2008, the SpaceX Falcon 1 Flight 3 executed a picture perfect first stage flight, ultimately reaching an altitude of 217 km, but encountered a problem just after stage separation that prevented the second stage from reaching orbit.

- komrad36: Wow! I have read about this flight and how Elon and team handled the aftermath, but I never thought there exists video of the residual thrust and collision! That's amazing!! And... with audio? Or was that added after for effect?

- Brian Bagnall: The collision at the end of the video happened because the first stage rocket burnt for an extra fraction of a second rather than shutting down completely. When separation occurred, it caused the first stage to accelerate into the second stage and they had a collision which damaged the second stage engine. Total failure.
